What Vitamins Should You Take On A GLP-1? A Plain Guide To The Nutrition Gap

Eating a third to a half less food on a GLP-1 means a third to a half less iron, B12, magnesium and zinc. Here is which nutrients are at risk, the protein target clinicians use, and what a companion supplement should contain.

On a GLP-1 medication the nutrients most worth watching are iron, vitamin B12, vitamin D, calcium and thiamine — the same list bariatric clinicians have monitored for decades, because the mechanism is the same: far less food going in.

Why does a GLP-1 create a nutrition gap?

Because food is not only calories. Semaglutide and tirzepatide work largely by slowing gastric emptying and acting on the brain's satiety signalling, which means you eat substantially less. In controlled feeding studies semaglutide cut energy intake by roughly a quarter, and a 60-week randomised trial published in 2026 found the reduction was still present a full year in.

Every plate you stop eating was also delivering iron, B12, magnesium, zinc and folate. Cut total food volume by 40% and, unless the composition of what remains changes deliberately, you cut those by roughly 40% too. That is arithmetic, not a side effect.

A 2025 review in the International Journal of Obesity described this as a nutrition guidance gap in GLP-1 care, and recommended borrowing protocols from bariatric surgery until GLP-1-specific evidence catches up.

Which nutrients are most at risk on semaglutide or tirzepatide?

Not everything falls at the same rate. What matters is how much reserve you carry, and how much the food you stopped eating was contributing.

How much protein do you need on a GLP-1?

The figure clinicians use is 0.8–1.6 g per kilogram of body weight per day, or 80–120 g in absolute terms — the same range proposed after bariatric surgery. The reason is body composition: reviews estimate 30–40% of the weight lost on GLP-1 therapy can be fat-free mass. Protein plus resistance training is the only lever that meaningfully changes that ratio.

On a plate half the size it used to be, order matters. Protein first, every meal, before anything else has a chance to fill the space.

What should a GLP-1 supplement actually contain?

If you take the guidance seriously, most of the supplement aisle is ruled out. Five criteria:

  1. Iron, in a chelated form. Ferrous bisglycinate rather than ferrous sulphate, which is the form responsible for iron's reputation for stomach upset. Most gummy multivitamins contain no iron at all, because iron tastes metallic.
  2. Active-form B vitamins. Methylcobalamin instead of cyanocobalamin, P-5-P instead of plain B6, L-5-MTHF instead of folic acid.
  3. Real magnesium and zinc, not token amounts.
  4. Something for digestion. Slowed gastric emptying is the drug's mechanism, not an inconvenience to argue with. Ginger, peppermint and digestive enzymes are unglamorous but well-established for that heavy, slow feeling.
  5. A format you can swallow on a bad stomach day. Two capsules with your largest meal survives contact with reality. A horse pill does not.

One honest caveat. No single capsule covers everything on the watch list. Copper, calcium and thiamine often need separate attention, and nothing replaces protein. A supplement is a floor, not a diagnosis.

When should you get bloodwork?

A reasonable starting panel is ferritin, vitamin B12, vitamin D and a full blood count. Most prescribers will run it if you ask directly. Ask again around the six-month mark, or sooner if fatigue, hair shedding or breathlessness appear. Numbers beat guessing, and they tell you whether a supplement is enough or whether you need something targeted.

Can I just take a normal multivitamin on a GLP-1?

You can, and it is better than nothing. Check two things: that it actually contains iron, and that the B12 is methylcobalamin rather than cyanocobalamin. Many one-a-day formulas fail the first test and almost all gummies do.

Does a supplement help with GLP-1 nausea?

Ginger has a long history of use for nausea and peppermint for digestive comfort, and both appear in formulas designed for this. Neither is a treatment for medication side effects — if nausea is severe or persistent, that is a conversation with your prescriber about dose titration.

Should I take vitamins before or after my GLP-1 injection?
